RATES AND INSURANCE
2018 RATES
- Intake/Assessment 60-75 min: $125
- Individual therapy 50 min: $90
- Individual therapy 75 min: $115
- Family therapy w/client 60 min: $115
- No show/late cancel fee (see policy below): $50
INSURANCE
I am currently in-network with Cigna, and am finalizing being in-network with Blue Cross/Blue Shield. You are responsible for your copay or co-insurance after paying your deductible.
Other insurance plans often have out-of-network benefits that are comparable to in-network benefits. I am happy to provide you with the necessary paperwork to file for reimbursement from you insurance company. You will be responsible for the full fee at the time of session, unless we make other arrangements. In many cases, I am able to file your paperwork for you.
Please verify your mental health benefits before your first session. You will need to know if you have a deductible, what your plan covers (number of sessions), if pre-authorization is needed, and what your out-of-network benefits are.
REDUCED FEES/SLIDING SCALE
Reduced fee services are available on a client by client basis. This should be discussed during your consultation or intake assessment.
PAYMENT
I accept cash, checks, credit cards, and HSA cards. Payment is due at the end of each session. There is a $25 fee for returned checks. If payment is an issue, it needs to be discussed before session.
CANCELATION POLICY
- Please provide 24 hours notice if you need to cancel your appointment, or you will be required to pay a $50 fee before your next scheduled appointment.
- If you miss more than 3 sessions, your case may be closed, and you will be referred to another provider.
- You will need to pay your late fee before you can schedule your next appointment.
